29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.    On February 2, 2018, Larry Herschberger dba PHD Construction, was issued citations and the employer did not verify, through documents that each citation was abated.  Abatement documentation was not received for the following violations:               a. Citation 1 Item 1;         b. Citation 2 Item 1;         c. Citation 2 Item 2; and         d. Citation 2 Item 3.   Larry Herschberger, dba PHD Construction, was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1903.19(d)(1) which was contained in OSHA inspection number 1211537, citation number 3, item number 2 and was affirmed as a final order on 07/03/2017, with respect to a workplace located at 722 Stone Bridge Center Drive, Rantoul, IL.

On or about November 28, 2017, Employees working off an elevated platform were exposed to a fall hazard on a construction site in Champaign, Illinois, and the employer did not ensure guardrails were installed along all open sides and ends of the platform.

On or about November 28, 2017, Employees were exposed to a struck by hazard in the eye, while using a pneumatic nail gun on a construction site in Champaign, Illinois, and the employer did not ensure the use of eye protection.         Larry Herschberger dba PHD Construction Was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(A)(1), which was contained in OSHA inspection number 978955 citation number 1, item number 1, and was affirmed as a final order on August 21, 2014, with respect to a workplace located at 68 E. Airport Road, Savoy, IL 61874.
