On November 28, 2017, the employer did not ensure an employee greasing the saw drive shaft bearings for the Wilson saw located in Plant 1 was trained on the company's lockout program. Training did not include the following:    a.    The recognition of applicable hazardous energy sources;    b.    The type and magnitude of the energy available in the workplace;    c.    The methods and means necessary for energy isolation and control

29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and were not done in the following sequence.    a. On November 28, 2017, the employer did not ensure an employee locked out the Wilson saw in Plant 2 prior to greasing the saw drive shaft bearings. The employee was exposed to rotational hazards of the drive shaft and the saw blade if the saw were to be energized.    b. On December 18, 2017, the employer did not ensure that the blade movement on the Park saw, located in Plant 2 was locked out when conducting set up activities.      c. On December 18, 2017, the employer did not ensure that the blade movement of the Wilson saw, located in plant 1 was locked out when conducting set up activities.    d. On November 28, 2017, the employer did not ensure that the blade movement on the GMM#2 saw in plant 1 was locked out when conducting set up activities.

29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not removed from service.     a.    On November 28, 2017, the employer did not ensure the Mitsubishi forklift located in plant 2 was removed from service.  During the inspection it was determined the forklift's emergency brake and horn were not functioning.      b.    On November 28, 2017, the employer did not ensure the Utilift forklift #UT30P located in plant 1 was removed from service.  During the inspection it was determined the forklift's emergency brake was not functioning.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service, and were placed in service when the examination showed any condition adversely affecting the safety of the vehicle. Such examinations were not made at least daily. Where industrial trucks are used on a round-the-clock basis, they were not examined after each shift. Defects when found were not immediately reported and corrected.  a.    On November 28, 2017, the employer did not ensure the Mitsubishi forklift located in plant 2 was inspected on a daily basis.  During the inspection it was determined the forklifts emergency brake and horn were not functioning.    b.    On November 28, 2017, the employer did not ensure the Utilift forklift #UT30P located in plant 1 was inspected on a daily basis.  During the inspection it was determined the forklifts emergency brake was not functioning.

29 CFR 1910.178(a)(5): If the truck is equipped with front-end attachments other than factory installed attachments, the user did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.    a.    On November 28, 2017, the employer did not ensure the Taylor forklift located in Plant 2 was equipped with a new data plate which showed the capacity and the load center of the manufactured boom used to lift large sections of sandstone.
