(a)       On or about September 8, 2017, the employer failed to submit documentation and certification of corrective action for citation #1, item #1 issued on December 18, 2017 with an abatement due date of December 29, 2017.   (b)       On or about September 8, 2017, the employer failed to submit documentation and certification of corrective action for citation #1, item #2a issued on December 18, 2018 with an abatement due date of December 29, 2017.   (c)       On or about September 8, 2017, the employer failed to submit documentation and certification of corrective action for citation #1, item #2b issued on December 18, 2018 with an abatement due date of December 29, 2017.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about September 8, 2017 at 304 Falling Leaf Lane, Canton, Georgia, an employee working on the roof installing shingles was exposed to a fall hazard of 10'-10" to the lower level.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about September 8, 2017 at 304 Falling Leaf Lane, Canton, Georgia, working on the roof installing roofing shingles, an employees were exposed to a fall hazard when the ladder they were using did not extend 3 feet above the working level.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.   On or about September 8, 2017 at 304 Falling Leaf Lane, Canton, Georgia, when mounting the ladder to access/egress the working level, an employee was exposed to a fall to the lower level.  The ladder used was missing a safety shoe.
