29 CFR 1910.95(k)(3): The employer did not ensure that each employee exposed  to noise at or above an 8-hour time weighted average of 85 decibels was trained and informed of the information included in sections k(3)(i) through k(3)(iii):  - k(3)(i): The effects of noise on hearing;  - k(3)(ii): The purpose of hearing protectors, the advantages, disadvantages, and attenuation of various types, and instructions on selection, fitting, use, and care;  - k(3)(iii): The purpose of audiometric testing, and an explanation of test procedures.    (a)    On or about January 31, 2018, the Trimmer working in the Middle Trim Booth was exposed to an 8-hour time weighted average noise level of approximately 98.0 dBA, which exceeded the Action Level of 85 dBA.   The training required by this standard had not been provided to the exposed employee.    (b)    On or about January 31, 2018, the Trimmer working in the West Trim Booth was exposed to an 8-hour time weighted average noise level of approximately 92.2 dBA, which exceeded the Action Level of 85 dBA.    The training required by this standard had not been provided to the exposed.    (c)    On or about January 31, 2018, the CNC Operator working at the DMS CNC machine was exposed to an  8-hour time weighted average noise level of 87.9 dBA which exceeded the Action Level of 85 dBA.   The training required by this standard had not been provided to the exposed employee.

29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:    (a) Prior to this inspection no medical evaluation for required respirator use was provided for employees working in the Middle Trim Booth and the West Trim Booth.  Air Supplied Hood Respirators were required in these areas for protection against exposure to fiberglass dust.

29 CFR 1910.1000(e): Feasible administrative or engineering controls were not determined and implemented to achieve compliance with the limits prescribed in 29 CFR 1910.1000(a) through (d):    (a) On or about January 31, 2018, the Trimmer working in the Middle Trim Booth was exposed to Fiberglass Dust as an 8-hour time weighted average of 32.4 mg/m3, approximately 2.16 times the limit of 15 mg/m3.  Existing control measures, including administrative and engineering controls, were not adequate to limit the exposure of employees to Total Dust to or below the PEL.    (b) On or about January 31, 2018, the Trimmer working in the West Trim Booth was exposed to Fiberglass Dust as an 8-hour time weighted average of 30.2 mg/m3, approximately 2.02 times the limit of 15 mg/m3. Existing control measures, including administrative and engineering controls, were not adequate to limit the exposure of employees to Total Dust to or below the PEL.

29 CFR 1910.134(d)(1)(ii): The employer did not select and use a NIOSH-certified respirator in compliance with the conditions of its certification:  (a) On or about January 31, 2018, the Trimmer working in the West Trim Booth was wearing an N95 filtering face piece dust mask inside of an Air Supplied Hood Respirator.  Although each of these respirators are NIOSH-certified, use of both respirators simultaneously was not in compliance with conditions of certification for either respirator.
