29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    a. On or about November 29, 2017, at the above referenced worksite, employees engaged in steel erection were working from a scaffold (scissor lift). An employee left the safety of the platform by standing on the guardrail, thereby defeating the effectiveness of said guardrail. The employer failed to ensure an alternative means of fall protection was used. The employee was thereby exposed to a fall of approximately 8' to concrete surface.     There is no abatement certification of documentation required for this item.
