29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace.    Tradition (on marine railway): Employee(s) required to use an elastomeric respirator, such as a full-face 3M with organic vapor cartridges, were not medically evaluated and cleared prior to using them for sanding and painting tasks. Employees were exposed to respiratory hazards.    South Coast Shipyard was previously cited for a violation of this occupational safety and health standard or its equivalent standard [29 CFR 1910.134(e)(1)], which was contained in OSHA inspection number 957734, citation number 1, item number 1A and was affirmed as a final order on March 31, 2014, with respect to a workplace located at 223 21st Street, Newport Beach, California 92663.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator, whenever a different respirator facepiece (size, style, model or make) was used, and at least annually thereafter.  Tradition (on marine railway):   a) Employee(s) required to use an elastomeric respirator, such as a full-face 3M with organic vapor cartridges, were not fit tested prior to using them for sanding and painting tasks. Employees were exposed to respiratory hazards.  b) Employee(s) required to use an elastomeric respirator, such as a full-face 3M with organic vapor cartridges, were not fit tested annually. Employees were exposed to respiratory hazards. 29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment.  Boatyard: A written hazard assessment had not been documented and certified, which included occupational tasks requiring personal protective equipment. Employees were exposed to eye, skin, and respiratory system injuries and illnesses.
