29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury: On or about November 27, 2017, and in times prior thereto, Iron workers were "free rigging" off the forks of a forklift for a below-the-forks lifting of bundles of metal decking. This type of lift does not use an approved lifting attachment, exposing employees to the hazards of being struck by metal decking.

29 CFR 1926.602(c)(1)(vi): Industrial truck(s) in use did not meet the applicable requirements of design, construction, stability, inspection, testing, maintenance, and operation, as defined in American National Standards Institute B56.1-1969, Safety Standards for Powered Industrial Trucks. Section 605 of the ANSI: Handle only stable or safely arranged loads: On or about November 27, 2017, and in times prior thereto, iron workers were "free rigging" off the tines of a forklift for a below-the-tine lifting of bundles of metal decking. Two nylon slings were placed one on each fork in an X pattern in order to swing around when pushed exposing workers to the hazard of being struck by metal decking.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about November 27, 2017, the employer did not ensure a worker on a steel joist approximately 28 feet high was protected from falls while free rigging operations of a bundle of metal decking were being performed in the area, exposing the worker to the hazard of falling through the holes located between each joist. The W.S. Steel Erection, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number 1109160, citation number 2, item number 2 and was affirmed as a final order on October 4, 2016, with respect to a workplace located at 198267 FM 306, Canyon Lake, Texas. The W.S. Steel Erection, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 886203, citation number 1, item number 1b and was affirmed as a final order on August 12, 2013, with respect to a workplace located at 1460 Precinct Line Road, Hurst, Texas.
