On or about 10/3/2018.    b) Machine Shop: One or more methods of machine guarding was not provided to protect the operator from hazards such as those created by rotating parts from a Varnsdorf Tos Horizontal Boring Mill Model#  WHT 05 CNC, SN: 0417. On or about 10/3/2018.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:      a) Facility: The employer did not provide hazard communication training to employees who used chemicals such as but not limited to ACE Premium Enamel and Rust-Oleu (spray paint). On or about 10/3/2018.           NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.     WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.  Gaum, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.1200(h)(1), which was contained in OSHA inspection number 1122499, citation number 2, item number 2 and was affirmed as a final order on 7/11/2016, with respect to a workplace located at 1080 US Hwy 130 Trenton, NJ 08691.
