29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of re-roofing operations. Workers were observed removing and installing shingles on a residential style construction site (house) without utilizing fall protection. The workers were exposed to falls of over 8 feet to concrete and grass below. This was most recently documented at 1715 Ada St., Grand Island, Nebraska on or about December 1st, 2017 when the employer had employees working on the roof of this residence without any form of protective system.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder. The employer is failing to ensure employees are protected from fall hazards while utilizing portable ladders as part of re-roofing operations. Workers were observed climbing up a portable extension ladder which did not meet the elements of this standard. Two ladders were not secured to prevent them from moving and sliding while workers are utilizing the ladder on this job site. The workers were exposed to falls of over 8 feet to dirt and concrete below. This was most recently documented at 1715 Ada St., Grand Island, Nebraska on or about December 1st, 2017 when the employer had employees working on the roof of this residence with the above listed issue present as they were going up and down the ladder on site.
