29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about December 4, 2017, employees were exposed to being struck by flying objects. The employees were performing roofing activities on a new residential home located at 36193 Harvest Court, North Ridgeville, Ohio 44035. The employees were using air powered nail guns without wearing protective safety glasses.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about December 4, 2017, employees were exposed to a fall of approximately 10 feet. The employees were performing roofing activities on a new residential home located at 36193 Harvest Court, North Ridgeville, Ohio 44035. The employees were using an extension ladder to access the roof that did not extend at least three feet above the landing area.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about December 4, 2017, employees were exposed to a fall of approximately 10 feet. The employees were performing roofing activities on a new residential home located at 36193 Harvest Court, North Ridgeville, Ohio 44035. The employees were working on all areas of the roof without using any form of fall protection. Madison Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 991328, citation number 1, item number 1 and was affirmed as a final order on 10/11/14, with respect to a workplace located at 8061 Columbia Rd., Olmstead Falls, Ohio 44138.
