29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about June 18, 2019 during residential roofing activities at a job site located at 7957 Burton Lane, Hudson, OH, employees were exposed to struck by hazards. The employees were using air powered nail guns without wearing protective safety glasses.   Madison Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1280658, citation number 01, item number 001 and was affirmed as a final order on January 10, 2018, with respect to a workplace located at 36193 Havencrest Ct., North Ridgeville, OH 44035.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On or about June 18, 2019 during roofing activities at a residential home located at 7957 Burton Lane, Hudson, OH employees were exposed to a fall of approximately 12 feet. The employees were working on all areas of the roof without using any form of fall protection.  Madison Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1280658, citation number 02, item number 001 and was affirmed as a final order on January 10, 2018, with respect to a workplace located at 36193 Havencrest Ct., North Ridgeville, OH 44035.  Madison Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(10), which was contained in OSHA inspection number 991328, citation number 1, item number 1 and was affirmed as a final order on 10/11/14, with respect to a workplace located at 8061 Columbia Rd., Olmstead Falls, Ohio 44138.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about June 18, 2019 during roofing activities at a residential home located at 7957 Burton Lane, Hudson, OH employees were exposed to a fall of approximately 12 feet. The employees were using an extension ladder to access the roof that did not extend at least three feet above the landing area.  Madison Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1280658, citation number 01, item number 002 and was affirmed as a final order on January 10, 2018, with respect to a workplace located at 36193 Havencrest Ct., North Ridgeville, OH 44035.
