29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed:    On or about December 3, 2017, the employer failed to ensure that a portable ladder the employees were using to access the rooftop of a commercial building (hotel) was extended at least three feet above the step off access point.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about December 3, 2017, the employer failed to ensure that employees who were working on a commercial roof were protected from a fall from height hazard of 30+ feet. Employees were wearing personal fall protection harnesses but none of them were tied off while up on the roof.
