29 CFR 1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Always Up Roofing, Inc. on or about July 18, 2023, at the location of a new homesite located at  the 33600 Block of Via San Angelo Drive, Avon, OH: The employer did not ensure that employees working at the ground level were protected from overhead hazards while roof work was being performed.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Always Up Roofing, Inc., on or about July 18, 2023,  at the location of a new homesite located at  the 33600 Block of Via San Angelo Drive, Avon, OH: The employer did not ensure that employees performing roofing activities on a steep roof were protected from fall hazards. Employees were exposed to fall hazards of 8-12 feet to the ground below while performing roofing activities.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   a) Always Up Roofing, Inc. on or about July 18, 2023, at the location of a new homesite located at  the 33600 Block of Via San Angelo Drive, Avon, OH: the employer did not ensure employees working on the roof of a multifamily dwelling were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material including with the use of pneumatic nail guns.    Always Up Roofing, Inc. was previously cited for a violation of this occupational safety and health standard(name previously cited standard, 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1403084, citation number 1, item number 1 and was affirmed as a final order on September 25, 2019, with respect to a workplace located at 32539 Lenox Dr, Fairview Park, Ohio 44126 .
