29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about May 22, 2019, during the installation of roofing material at the job site located at 32539 Lenox Dr, Fairview Park, Ohio 44126; the employer did not ensure employees working on the roof of a commercial location (multifamily dwelling) were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   On or about May 22, 2019, during the installation of roofing material at a job site located at 32539 Lenox Dr, Fairview Park, Ohio 44126; the employer did not ensure employees working on a roof of a commercial location were protected from a fall of approximately 9.2 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about May 22, 2019, at the job site located at 32539 Lenox Dr, Fairview Park, Ohio 44126; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.   The Always Up Roofing Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(1) Duty to have fall protection), which was contained in OSHA inspection number 1280698, citation number 01, item number 01B and was affirmed as a final order on (2/21/2018), with respect to a workplace located at 9459 SR 14, Streetsboro, Ohio 44241.
