This was most recently documented at a worksite located at or near 2019 S. 219th St., Elkhorn, Nebraska 68022 on or about December 5, 2017.The employer did not ensure that adequate fall protection was provided for and worn at all times while an employee was working on the frame of a home that was approximately 11 feet from a lower level.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: The employer is failing to protect employees from fall hazards. This was most recently documented at a worksite located at or near 2019 S. 219th St., Elkhorn, Nebraska 68022 on or about December 5, 2017.The employer is failing to ensure that employees were using a ladder for the purpose for which it was designed. A) An employee was using a portable A-frame ladder to view an upper landing surface while framing a house. The employee was using the A-frame ladder leaned against the house in a closed position. B) An employee was using the A-frame ladder in closed position and standing on the top step. Abatement certification is required for this violation. Abatement certification sheet is enclosed with the citation.
