29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: Employee(s) were exposed to fall hazards. This was observed on or about September 3, 2025, at 21718 Hackberry, Gretna, Nebraska. Employees were exposed to a fall hazard in that a supported scaffold platform was being used with a JLG powered industrial truck was not secured to the forks.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: Employee(s) were exposed to fall hazards. This was observed on or about September 3, 2025, at 21718 Hackberry, Gretna, Nebraska. Employees were exposed to a fall hazard in that employees were not utilizing fall protection while working on a JLG mobile scaffold that was elevated to a height greater than 10 feet above the next lower level.
