29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) 110 N Bridge Street, Holyoke, MA: On or about December 5, 2017 powered industrial trucks which were in need of repair, or in any way unsafe had not taken out of service until restored to safe operating condition. (a) Komatsu fork truck, Model FGC40K, Serial # AF87A00190, had a horn which did not function when depressed (b) Mitsubishi fork truck, Truck #3, had an operator's seat which was severely damaged and was missing a locking pin from the right fork.

29 CFR 1910.254(d)(9)(iii): Cables with damaged insulation and/or exposed bare conductors were not replaced: 110 N Bridge Street, Holyoke, MA: On or about December 5, 2017 employees were using welding equipment which had its insulation damaged on the ground cable and missing where the lead output lugs meet the welder, exposing employees to contact with bare conductors.

29 CFR 1910.303(g)(2)(i):Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures: 110 N Bridge Street, Holyoke, MA: On or about December 5, 2017 employees were exposed to electrical burns and shocks from live electrical parts operating at greater than 50 volts and were not guarded against accidental contact. (a) Pendant control, Gantry crane, control panel emergency stop button was missing from pendant and parts below were live. (b) Box, 4 receptacle, power source for bench grinder. Ceramic or plastic receptacle faces were heavily damaged.
