29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:    a) On or about December 5, 2017, at the above address jobsite, in the employee room in the Biscayne Unit (FCI), visible mold in the ceiling and wall was present.     b) On or about June 1, 2018, at the above address jobsite, in the Education Building (FCI), Room E1-018, visible mold in the wallpaper was present.    The employer was cited for a violation of the same standard, 1910.22(a)(1) on March 6, 2017, Inspection #1178558, with a final order/settlement date of April 4, 2017.

29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition:  On or about December 5, 2017, at the above address jobsite, in the Biscayne unit (FCI), a section of the ceiling was leaking, therefore exposing employees to a slip and fall hazard.  The employer was cited for a violation of the same standard, 1910.22(a)(2) on March 6, 2017, Inspection #1178558, with a final order/settlement of April 4, 2017.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days  of receiving information that a recordable injury or illness has occurred:     On or about June 1, 2018, at the above addressed worksite, the employer failed to record a recordable injury on either the log of all Work-Related Injuries and Illnesses (OSHA Form 300), and/or the Injury and Illness Incident Report (OSHA Form 301), or their equivalents, work-related injuries or illnesses for the calendar year 2018:  On March 16, 2018, a employee sustained rib injuries. As a result, the employee received both medical treatment beyond first aid in the form of prescription medication and days away from work.

29 CFR 1960.8(d): The head of each agency did not acquire, maintain, and require the use of approved personal protective equipment, approved safety equipment, and other devices necessary to protect employees.    a) On or about June 1, 2018, and at times prior, the employer failed to require that augmented employees wore the same level of protective footwear (steel toe or reinforced toe) as required for correctional officers when augmented staff were assigned custody duties while covering correctional officers' shifts during augmentation.
