29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about December 4, 2017 employees performing sewer and water installation operations were exposed to struck by hazards when employees worked on two different levels when the employer failed to mandate the use of hard hats. No abatement certification or documentation is required for this item.

29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section except when: On or about December 4, 2017, employees performing sewer and water installation at a commercial property in a 13.5 foot deep trench were exposed to cave-in hazards when the employer failed to ensure the employees were protected by an adequate cave-in protection system. No abatement certification or documentation is required for this item.
