29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about June 18, 2019, employees were not provided with a safe means for entering and exiting a 4.5 foot deep trench, exposing them to fall hazards. No abatement documentation or certification is required of this item.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about June 18, 2019, employees were not wearing hard hats while working in a trench where overhead hazards and impact injuries from an operating excavator exposed them to struck-by hazards. Aria Sewer & Water LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.100(a), which was contained in OSHA inspection number 1281485, citation number 1, item number 1 and was affirmed as a final order on March 13, 2018, with respect to a workplace located at 1558 Ogden Ave., Naperville, IL 60540. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
