(b) Outside Maintenance Shop: On or about December 6, 2017 and times prior, employees operated the left grinding wheel of a Buffalo Heavy Duty Bench Grinder (Serial No. 5448) that had a 1/2 inch opening between the tongue guard and the abrasive wheel periphery.
