29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:    a) Bunting Bearings, Inc. located at 200 Van Buren Street in Delta, Ohio; On or about December 8, 2017 the employer did not ensure employees traveling to and from the water room located beneath the casting deck were protected from fall hazards in that the guardrails surrounding pits #4 and #5 had been removed. Employees were not utilizing any form of fall protection and were exposed to falls of approximately ten feet.    b) Bunting Bearings, Inc. located at 200 Van Buren Street in Delta, Ohio; On or about April 26, 2018 the employer did not ensure employees removing steel bars from Pit #2 were protected from fall hazards. Employees removing cut steel bars from below Machine #2 were not utilizing any form of fall protection and were exposed to falls of approximately ten feet.

29 CFR 1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:    Bunting Bearings, Inc. located at 200 Van Buren Street in Delta, Ohio; On or about December 8, 2017 the employer did not ensure employees who were required to wear tight fitting elastomeric half mask respirators were clean shaven. An employee adding alloy to the furnace located on the casting deck was exposed to hazards associated with lead and metal fumes such as, but not limited to, fatigue, abdominal pains, and anemia.

Employees entering the pits of the vertical casting machine located in the centrifugal department to install the repaired vertical caster were exposed to potential hazardous atmospheres and water engulfment hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:    a) Bunting Bearings, Inc. located at 200 Van Buren Street in Delta, Ohio; On or about April 20, 2018 the employer did not ensure employees were trained in the purpose and function of the energy control program. Employees were required to enter the mold holding pits located on the front of the vertical centrifugal casting machines to anchor the mold to the alignment plate. Employees were not adequately trained in the proper lock out procedures exposing them to caught in hazards from the spinning alignment plate.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a) Bunting Bearings, Inc. located at 200 Van Buren Street in Delta, Ohio; On or about April 20, 2018 employees were exposed to caught in hazards in that the vertical centrifugal casting machine located in the centrifugal department was not locked out prior to anchoring the mold to the alignment plate. The employer did not ensure that energy control application steps to control the electrical energy sources were implemented prior to setting the mold. The employer did not:    -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe.  -  Verify that isolation and de-energization of the machine or equipment had been accomplished.
