This most recently occurred on December 8, 2017 on the jobsite located at 21518 Parkview Dr, Gretna, NE 68028.  Four employees were re-roofing a two-story house with a six in twelve steep sloped pitch at heights greater than 6 feet above the ground and not utilizing any form of fall protection.  An employee fell approximately seventeen feet from the eve of the roof to the ground sustaining fatal injuries.
