29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  On or about October 16, 2017, employees were exposed to the hazard of contact with stored energy, while conducting maintenance and servicing on equipment such as but not limited to:  ball valves, check valves, and 2-way valves within the processing lines in the Brew House and Dryer Room and adequate training was not provided in accordance with the standard.

29 CFR 1910.147(f)(3)(i): When servicing and/or maintenance is performed by a crew, craft, department or other group, they shall utilize a procedure which affords the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.  On or about October 16, 2017, employees were exposed to the hazard of contact with stored energy, while conducting maintenance and servicing on equipment such as but not limited to: ball valves. check valves, and 2-way valves within the processing lines in the Brew House and Dryer Room and a procedure which afforded an equivalent level of protection to personal lockout was not implemented.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.    The employer did not conduct periodic inspections of the energy control procedures to ensure an accurate procedure and employees (such as but not limited to maintenance employees) were familiar with their responsibilities under the procedures.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    Employees performing servicing and maintenance activities were not required to:  (1)  know the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy,  (2)  turn off and shut down the machines  (3)  physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4)  affix lockout or tagout devices to each energy isolating device,  (5)  prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.    (a)  On or about December 3, 2017, in the Brew House, the XV3135 Utility Water Valve was removed from the pipe system while the line was energized exposing employees to being sprayed with hot water.    (b)  On or about October 13, 2017, in the Dryer Room, the RC111 2-way valve was removed from the pipe without releasing pressure in the pipe system exposing employees to being sprayed with hot water.

On or about October 16, 2017 and thereafter, the employer did not document and utilize energy control procedures when performing maintenance and servicing on equipment such as but not limit to: ball valves, check valves, and 2-way valves within the processing lines in the Brew House and Dryer Room.
