29 CFR 1926.502(d)(10)(i): Except as provided in paragraph (d)(10)(ii) of this section, when vertical lifelines were used, each employee was not attached to a separate lifeline: a) 151 Columbia Turnpike, Florham Park, NJ. Subcontractor employees were exposed to falls of up to 17 feet from the roof of the structure undergoing demolition as at least 3 workers were tied off to a single rope lifeline on the roof, which would increase the chances for all of the workers to fall given that one fell and would increase the chances for a lifeline failure during a fall. Violation observed on or about 12/18/2017.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) 151 Columbia Turnpike, Florham Park, NJ. Subcontractor employees were exposed to falls of up to 17 feet from the roof of the structure undergoing demolition as fall protection equipment was rigged with excess rope of the lifeline at the workers' feet which would cause a worker to free fall more than 6 feet and hit the ground before the system stopped him or result in high deceleration forces on the worker. Violation observed on or about 12/18/2017.
