Employees were required to climb within the machine to clean rollers, thread the spool of paper, and repair paper web tears without the machine being locked out to prevent inadvertent activation of the various rollers, actuators and calendars. This condition exposed employees to ingoing nip points, rotating parts, and amputation hazards.

Operations employees performed periodic cleaning and repair of web tears on the HUD-09 machine without following the company's lockout tag out procedure.  The employer did not verify through periodic inspections that employees utilized the energy control procedure.  This condition exposed employees to ingoing nip points, rotating parts, and amputation hazards.    Abatement note:  29 CFR 1910.147(c)(6)(ii) provides that the employer shall certify that the periodic inspections have been performed.  The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

The insulation jacket had exposed inner wires which exposed employees to an electrical shock hazard.
