29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Hexcel Corporation located at 31815 Great Western Drive, Windsor, CO 80550:  On and preceding December 12, 2017, the employer did not ensure that procedures were utilized for the control of potentially hazardous energy when employees were engaged in cleaning, threading, and repair of paper web tears on the HUD-09 machine.  Employees were required to climb within the machine to clean rollers, thread the spool of paper, and repair paper web tears without the machine being locked out to prevent inadvertent activation of the various rollers, actuators and calendars. This condition exposed employees to ingoing nip points, rotating parts, and amputation hazards.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    a)  Hexcel Corporation located at 31815 Great Western Drive, Windsor, CO 80550:  On and preceding December 12, 2017, the employer did not ensure that an annual or more frequent inspection of the energy control procedure for the HUD-09 machine was performed to ensure that the procedure and requirements of this standard were followed.  Operations employees performed periodic cleaning and repair of web tears on the HUD-09 machine without following the company's lockout tag out procedure.  The employer did not verify through periodic inspections that employees utilized the energy control procedure.  This condition exposed employees to ingoing nip points, rotating parts, and amputation hazards.    Abatement note:  29 CFR 1910.147(c)(6)(ii) provides that the employer shall certify that the periodic inspections have been performed.  The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tag out device when servicing and/or maintenance was performed by a crew, craft:    a) Hexcel Corporation located at 31815 Great Western Drive, Windsor, CO 80550:  On and preceding December 12, 2017, the employer did not ensure that group hazardous energy control procedures were utilized when cleaning and working within the HUD-09 machine.  Multiple operations employees climbed within the machine at the same time, without using a group lockout procedure, to clean rollers, thread the spool of paper, and repair paper web tears.  This condition exposed employees to ingoing nip points, rotating parts, and amputation hazards.

29 CFR 1910.334(a)(2)(i):  Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):  a)  Hexcel Corporation located at 31815 Great Western Drive, Windsor, CO 80550:  On and preceding December 12, 2017, the employer did not ensure that portable cord and plug connected equipment and flexible cord sets were visually inspected before use on any shift for external defects. The Curis BOM 101 electric hand cutting machine was used to cut samples of material 30-40 times per day.  The insulation jacket had exposed inner wires which exposed employees to an electrical shock hazard.
