On or about December 13, 2017, employees operating the Galvanizer 2 line were exposed to an unguarded beater shaft (twelve inches from the catwalk) while opening the guard railing gate and adding water to the rinse tank.

29 CFR 1910.219(f)(3): Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more than seven (7) feet above the floor or platform. Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets. a. On or about December 13, 2017, employees operating the Galvanizer 2 line were exposed to a rotating chain and sprocket (approximately four feet from the floor) at the 480 volt drive motor as a result of an approximately three inch opening in the expanded metal guard. b. On or about December 13, 2017, employees unloading chain link fencing from the Galvanizer 2 line were exposed to a rotating chain and sprocket, approximately six foot from the ground, while bending the end of and pulling the breakdown picket.
