29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: On or about 14 December 2017, and at times prior thereto, a roofer was performing roofing activities on a two-story roof under construction with too much slack in the safety line, exposing the roofer to a fall hazard of approximately 19 feet 10 inches to the concrete slab below.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about 14 December 2017, and at times prior thereto, a roofer was installing wooden slats on a roof with a pneumatic nail gun without wearing eye protection, exposing the employee and another nearby to the hazard of being struck by flying objects or debris. Alfredo Vences was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1156494, citation number 01, item number 002 and was affirmed as a final order on 07/11/2016, with respect to a workplace located at 9107 Bayshore Bend, Austin, TX 78726.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about 14 December 2017, and at times prior thereto, two roofers accessed the 1st story roof with an aluminum extension ladder that was setup approximately 1 to 1.5 feet above the roof edge, exposing the roofers to the hazard of falling ~10 feet to the ground below. Alfredo Vences was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1156494, citation number 01, item number 004A and was affirmed as a final order on 07/11/2016, with respect to a workplace located at 9107 Bayshore Bend, Austin, TX 78726.
