29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. This violation was observed on or about December 12, 2017 at 8336 S. Lewis Ave, Tulsa, Oklahoma the employer did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the Genie S-80X Articulating Boom Aerial lift basket exposing employees to a fall from approximately 40 ft. to the ground below.

29 CFR 1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. This violation was observed on or about December 11, 2017 at 8336 S. Lewis Ave, Tulsa, Oklahoma, employees engaged in roofing activities from a low slope roof with an unprotected edge were not using any method of a fall protection system. This exposed the employees to a fall greater than six (6) feet to the ground below.
