This violation was observed on or about December 12, 2017 at 8336 S. Lewis Ave, Tulsa, Oklahoma the employer did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the Genie S-80X Articulating Boom Aerial lift basket exposing employees to a fall from approximately 40  ft. to the ground below.

This violation was observed on or about December 11, 2017 at 8336 S. Lewis Ave, Tulsa, Oklahoma, employees engaged in roofing activities from a low slope roof  with an unprotected edge were not using any method of a fall protection system.  This exposed the employees to a fall greater than six (6) feet to the ground below.
