29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 12, 2017 , and at times prior to, the employer failed to ensure that employees engaged in greasing the drive chain in the kill floor area did so utilizing a procedure to control potentially hazardous energy associated within the start up of the drive chain.

29 CFR 1910.147(d): 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) covered the following elements, however, the actions were not done in sequence as required by 29 CFR 1910.147 (d)(1)-(6): On or about December 12, 2017, and at times prior to, employees were exposed to machine hazards associated with moving parts and in-going nip points when they engaged in greasing of a drive chain in the kill floor area. The employer did not ensure that employees implemented energy control application steps prior to allowing them to grease the drive chain. The drive chain was not shut down or turned off prior to engaging in this servicing and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
