29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a) Craters & Freighters at 500 Burbank Street Suite C, Broomfield, CO 80020:  On and before December 20, 2017, the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body.  Employees worked with chemicals including Black and Red Ink, Spray Paint, Lithium Grease Spray, Silicone Lubricant Spray, Adhesive Remover, De Icer, Turpentine, 700 PVC, Killz Spray, Spray Adhesive, and Teflon Spray.  In addition, employees added fluids to forklift batteries.  The employer did not provide an eyewash for employee use.  This condition exposed the employees to the risk of permanent damage to the eyes.    Abatement Note:  A feasible means of abatement includes but is not limited to: 1) a portable eye wash device, appropriate for the expected hazardous exposure, and containing not less than one gallon of potable water; or 2) a plumbed emergency eyewash unit capable of delivering not less than 1.5 liters (0.4 gallons) of clean water per minute, with flow valves that are simple to operate, go "on" in one second or less, and remain "on" during the eyewash procedure.  For details on emergency eyewash and shower equipment, OSHA references the American National Standard for Emergency Eyewash and Shower Equipment, ANSI Z358.1.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    (a) Craters & Freighters at 500 Burbank Street Suite C, Broomfield, CO 80020:  On or before December 20, 2017, the exposing employer did not ensure that employees working in forklifts were trained to operate the lift.  The employer did not train and evaluate each operator who is authorized to operate the A3EC2-40200-up Caterpillar forklift to ensure they were competent to operate the forklift safely. This exposed employees to potential struck by and struck against hazards.

29 CFR 1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":  a)  Craters & Freighters at 500 Burbank Street Suite C, Broomfield, CO 80020:  On or before December 20, 2017, the employer did not have each emergency exit marked by a clearly visible sign that read "Exit". There was no exit sign located on the south door of the warehouse building. This condition exposes the employees to hazards associated with fire and smoke.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to annual maintenance checks.  Note:  Applies to any extinguishers provided in the workplace.  Maintenance check includes an internal examination, except extinguishers with stored pressure:  a) Craters & Freighters at 500 Burbank Street Suite C, Broomfield, CO 80020:  On or before December 20, 2017, the employer did not ensure that portable fire extinguishers were inspected at least annually in that the extinguishers in the facility had not been inspected since 2014.  Fire extinguishers were provided for employee use in fighting incipient fires.  The site included combustible materials including wood and flammable liquids.  This condition exposed the employees to fire hazards.

Fire extinguishers were provided for employee use in fighting incipient fires.  The site included combustible materials including wood and flammable liquids.  This condition exposed the employees to fire hazards.
