29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks.      (a) Craters & Freighters at 500 Burbank Street Suite C in Broomfield, CO 80020:  On and before December 20, 2017, the employer did not develop, implement, and maintain a written hazard communication program which at least described how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also included a hazardous chemicals list and methods used to inform employees of the hazards of non-routine tasks.  Chemicals used at this site include Black and Red Ink, Spray Paint, Lithium Grease Spray, Silicone Lubricant Spray, Adhesive Remover, De Icer, Turpentine, 700 PVC, Killz Spray, Spray Adhesive, and Teflon Spray. This condition exposes employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.

(a) Craters & Freighters at 500 Burbank Street Suite C, Broomfield, CO 80020: On and before December 20, 2017, the employer did not maintain a safety data sheet for each hazardous chemical, lubricant, flammable liquid, and combustible liquid stored in the warehouse. These chemicals include Black and Red Ink, Spray Paint, Lithium Grease Spray, Silicone Lubricant Spray, Adhesive Remover, De Icer, Turpentine, 700 PVC, Killz Spray, Spray Adhesive, and Teflon Spray. This condition potentially exposes employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  (a) Craters & Freighters at 500 Burbank Street Suite C in Broomfield, CO 80020: On or before December 20, 2017, the employer did not provide information and training to employees regarding the hazardous chemicals used in their work areas. 29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):   a) Craters & Freighters at 500 Burbank Street Suite C Broomfield, CO 80020:  On or before approximately December 20, 2017, the employer did not perform a personal protective equipment hazard assessment for the job duties performed at this site.  The employer did not have a written personal protective equipment hazard assessment in the facility for each job task that warranted the use personal protective equipment.  This condition exposed employees to physical and chemical hazards at the site.
