29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. a. On or about December 4, 2017, the employer did not ensure that periodic inspections of lockout/tagout procedures were conducted at least annually on machines throughout the facility, thereby exposing the employees to the unexpected start of equipment and release of stored energy.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about December 4, 2017, located at Wolverine Flexographic Printing Press #3, employees were exposed to machine hazards associated with moving parts and ingoing nip points while inspecting, cleaning plates, troubleshooting, repairing popups and performing setup activities, i.e. changing cylinders. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. b. On December 4, 2017, located at Wolverine Flexographic Printing Press #3, the employer did not ensure an employee was protected from the ingoing nip point of Wolverine Flexographic Printing Press #3 while performing the lockout/tagout activity of inspecting the printing press. The employee was holding a rag while inspecting a press after a barrier guard had been removed causing the rag and employee's fingers to be pulled into the ingoing nip point between the anilox and print cylinder resulting in a finger amputation.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a. On or about December 4, 2017, located on Wolverine Printing Press #3, there was a barrier guard over the ingoing nip point of the rollers that was not affixed to the machine and could be removed by hand, thereby exposing the employees to a caught-in hazard.

29 CFR 1910.219(f)(1): Gears. Gears shall be guarded in accordance with one of the following methods: (i) By a complete enclosure; or (ii) By a standard guard as described in paragraph (o) of this section, at least seven (7) feet high extending six (6) inches above the mesh point of the gears; or (iii) by a band guard covering the face of gear and having flanges extended inward beyond the root of the teeth on the exposed side or sides. Where any portion of the train of gears guarded by a band guard is less than six (6) feet from the floor a disk guard or a complete enclosure to the height of six (6) feet shall be required. a. On or about December 20, 2017, located in the Printing Department at Wolverine Printing Press #3, there were unguarded portions of gears that were approximately 25 to 34 inches above the floor, thereby exposing employees to a caught-in hazard.
