29 CFR 1926.502(f)(1): Warning lines were not erected around all sides of the roof work area: (a) Roof over B Section - The warning line system used as fall protection did not comply with the standard in that the warning line did not extend along the entire west edge of the roof and did not extend for about half the length of the east edge, exposing employees to the hazard of falling over 11.5 feet to the ground below, on or about December 21, 2017. (b) Roof over C Section - The warning line system used as fall protection did not comply with the standard in that the two employees working at the south edge of the roof took down the warning line in order to install the first row of roofing materials along this edge, exposing employees to the hazard of falling 11.5 feet to the ground below, on or about December 21, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.502(f)(1)(ii): When mechanical equipment was being used, the warning line was not erected at least 6 feet (1.8 m) from the roof edge which was parallel to the direction of mechanical equipment operation, and at least 10 feet (3.1 m) from the roof edge which was perpendicular to the direction of mechanical equipment operation: (a) Roof over C Section - Employees used a manual pull cart (human propelled wheeled cart) to move materials east and west on the roof. The warning line system used as fall protection did not comply with the standard in that employees erected the warning line for the west edge of the roof right at the edge (an edge perpendicular to the direction of the pull cart travel) rather than 10 feet back from this edge and erected the warning line for the north edge of the roof right at this edge (an edge parallel to the direction of the pull cart travel) rather than 6 feet back from the edge, exposing employees to the hazard of falling 11.5 feet to the ground below, on or about December 21, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

(a) Roof over C Section - The warning line system used as fall protection did not comply with the standard in that along the north edge of the roof, the east end of the warning line sagged down to where the flags touched the roof surface, exposing employees to the hazard of falling 11.5 feet to the ground below, on or about December 21, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.503(c)(3): When inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding or skill, the employer did not retrain the affected employee: (a) Roofs over B and C Sections - Employees required retraining in the use and installation of warning line systems as evidenced by the facts that employees did not erect warning lines along all sides of the roof work area, removed warning lines to install roofing materials at the edge, erected warning lines right at the roof edge, and allowed the warning line to sag, exposing employees to the hazard of falling 11.5 feet to the ground below, on or about December 21, 2017. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
