29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)      On or about December 8, 2017, employees performing roofing activities were exposed to a fall hazard on a construction site in Lincoln, IL and the employer did not ensure a competent person inspected the job site for unsafe activity and took necessary steps to eliminate or control fall hazards.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:      On or about December 8, 2017, employees performing roofing activities were exposed to a fall hazard of approximately 20ft. on a construction site in Lincoln, IL and the employer did not ensure the use of fall protection.

29 CFR 1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee:       On or about December 8, 2017, the employer did not maintain documentation of the required training for each employee who might be exposed to fall hazards.  There was no written documentation that included the employees name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
