Saint Marys, OH: The employer did not ensure the guardrail system installed along the elevated platform surrounding the Low Pressure Molding (LPM) #25 Machine was adequately secured against displacement. Employees working from the elevated platform in this condition were exposed to a fall of 7ft. 4 in. to the concrete floor below.
