On or about December 21, 2017, at the above addressed jobsite, employees conducting roofing operations were not provided training on how to minimize fall protection hazards, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.    a.  On or about December 21, 2017, at the above addressed jobsite, employees using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):       a.  On or about December 21, 2017, at the above addressed jobsite, employees performing residential sheeting installation operations, twenty (20) feet four (4) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.      ELLIOTT CARPENTRY AND CONTRACTING LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 906168, citation number 1, item number 1 and was affirmed as a final order on June 19, 2013, with respect to a workplace located at Foxborough Estates lot 261, 195th Street, Mokena, IL.      ELLIOTT CARPENTRY AND CONTRACTING LLC was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 853203, citation number 1, item number 1 and was affirmed as a final order on February 26, 2013, with respect to a workplace located at 4536 Franklin, Western Springs, IL.
