On or about December 12, 2017 through December 22, 2017 machine guarding was not adequate to protect employees from contacting the log shear blade when removing densified foam logs by hand at the exit end of the machine.  Employees were exposed to laceration hazards.  There is no abatement certification or documentation required for this item.

Recycling Room, Bright Technologies EPS Foam Densifier Machine (SN: DN11 02 10514, MN: D120).  On or about December 12, 2017, the log shear guard flip door at the exit end of the machine was not properly installed, creating a hazard in that the guard could be lifted upwards and then fall, striking the operator in the hands/fingers when removing densified foam logs by hand.  Employees were exposed to fracture and laceration hazards.     There is no abatement certification or documentation required for this item.
