The employer is failing to protect employees from fall hazards and struck-by hazards associated with residential roofing activities. This was most recently documented on or about December 18, 2017 at a jobsite located at Lot 9, Block Q, Waters Edge St. 1100 E., Derby KS 67037.  Employees engaged in residential roofing activities were exposed to fall hazards and struck-by hazards in that the employer had not developed and implemented safety and health programs that includes but is not limited to: working at heights greater than 6 feet, ladder safety, personal protective equipment, powered hand tool safety, and housekeeping.

The employer is failing to protect employees from puncture hazards and slip and fall hazards associated with residential construction activities.  This was most recently documented on or about December 18, 2017 at a jobsite located at Lot 9, Block Q, Waters Edge St. 1100 E., Derby, Kansas 67037.  Employees engaged in residential roofing activities at the ground level were exposed to puncture and slip and fall hazards in that debris, including debris with protruding nails, was not kept cleared from the walking and working areas.

The employer is failing to protect employees from fall hazards associated with the use of portable metal ladders while engaged in residential roofing activities.  This was most recently documented on or about December 18, 2017 at a jobsite located at Lot 9, Block Q,  Waters Edge St. 1100 E. Derby, Kansas 67037.  Employees engaged in residential roofing activities, using portable metal ladders, were exposed to fall hazards of approximately 10 and 16 feet from the eave to the ground below in that the ladders being used did not extend at least 3 feet above the eave and were not secured.   a) The Louisville 20 foot aluminum portable extension ladder, model W-2328-20, duty rating 200lbs., at the front of residence.  The fall height was approximately 10 feet from the eave to the ground below.    b) The Werner 28 foot aluminum portable extension ladder, model D1328-2, duty rating 250 lbs., at the rear of residence.  The fall height was approximately 16 feet from the eave to the ground below.
