On or about December 27, 2017, a carpenter employee using a Dewalt Circular Saw to cut blocks was exposed to an eye injury hazard in that he was not wearing protective eyewear while exposed to flying debris. b. On or about December 27, 2017, a carpenter employee installing blocks in the trusses above the garage with a pneumatic nailer was exposed to an eye injury hazard in that he was not wearing protective eyewear.

On or about December 27, 2017, a carpenter employee installing blocks in the trusses above the right-side garage was exposed to a 12 feet fall hazard to the concrete floor in that he was not protected by a fall protection system.
