This was most recently documented on December 27, 2017 at 7009 S 42nd St, Bellevue, Nebraska where employees were repairing a sewer pipe in an excavation that had undermined walls.  The undermined walls exposed employees to a struck by hazard from soil and concrete above them, and an excavator was parked above one of the undermined walls of the excavation.

The employer allowed the employees to enter the excavation without any form of protective system in place.
