29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: Employees engaged in production activities with the use of a flat press were exposed to crushing, caught in between, and amputation hazards in that guarding was not provided to prevent employees from reaching the point of operation.
