29 CFR 1910.212(a)(3)(iii): Special hand tools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: (a) Press Area: On or about January 3, 2018 the porta bars used to handle hot metal pieces in the forging presses and upsetters were used in a manner that placed the employees' hands within a few inches of the pinch points of each press in that employees were holding onto the porta bar with one hand and the piece being forged with the other hand while the press cycled. (b) Press Area: On or about January 19, 2018 employees were not using any hand tools to handle the shorter hot metal pieces in the forging presses in that employees were holding onto the shorter pieces being forged with only their hand while the press cycled which placed the employee's hand within a few inches of the pinch points of each press.

(a) On or about 12/28/17 the employer had not recorded each recordable injury and illness on the OSHA-300 log including, but not limited to, a lost time injury that occurred on December 6, 2017 when an employee was seriously injured at the 1300 press.
