29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program which at included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii): (Construction Reference: 1926.59)      Employees engaged in demolition and maintenance activities were exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals.  The employer had not developed or implemented a written hazard communication program to ensure employees were aware of the hazards associated with the chemicals with which they work throughout demolition activities.  Hazardous chemicals used throughout the daily activities by the employees include but are not limited to automotive fluids and lubricants.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (Construction Reference: 1926.59)     Employees engaged in demolition and maintenance activities were exposed to injury/illness from dermal, ingestion, and inhalation hazards from exposure to hazardous chemicals.  Employees had not been provided with training or information regarding the hazardous chemicals with which they work.  The employees had not been trained on how to read and understand safety data sheets or where the safety data sheets are located in order to review as needed. Hazardous chemicals used throughout the daily activities by the employees include but are not limited to automotive fluids and lubricants.

29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations.      At a jobsite located at 740 NW Blue Parkway, Lees Summit, Missouri: Employees were exposed to serious struck by and caught in hazards in that no engineering survey was performed to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. Employees conducted demolition operations in the areas surrounding multi story elevator shafts after portions of the building had been removed, and operated machinery on and around a floor covering the below-grade basement level of the building after portions of the building had been removed.
