29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a. Jones Potato Chip Co. - Mansfield, Ohio: On or about December 6, 2017, the employer did not develop, document, and utilize energy control procedures for the bagging machines.  Employees that set-up, unjam, and repair the machines were exposed to injury in that energy control was not utilized for servicing and maintenance tasks.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):    a. Jones Potato Chip Co. - Mansfield, Ohio: On or about December 6, 2017, employees were exposed to pinch point and rotating part hazards in that the #4 Hayssen bagging machine (s/n 086013) was not deenergized prior to servicing tasks such as, but not limited to, unjamming the machine.  The employer did not ensure that energy control application steps were implemented to control the machine's electrical/mechanical and pneumatic energy sources prior to the servicing task - the employer did not:   -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Verify that isolation and deenergization of the machine or equipment had been accomplished.    b. Jones Potato Chip Co. - Mansfield, Ohio: On or about January 10, 2018, employees were exposed to pinch point and rotating part hazards in that the #1 Hayssen bagging machine was not deenergized prior to servicing tasks such as, but not limited to, threading and replacing the belts on the machine.  The employer did not ensure that energy control application steps were implemented to control the machine's electrical/mechanical and pneumatic energy sources prior to the servicing tasks - the employer did not:   -  Ensure employees had the method or means to control hazardous energy.  -  Ensure that equipment was turned off or shut down.  -  Physically operate energy isolating devices to control energy sources and affix lockout device(s).  -  Verify that isolation and de-energization of the machine or equipment had been accomplished.

Jones Potato Chip Co. - Mansfield, Ohio: On or about December 6, 2017, the employer did not ensure that employees who setup and maintain the bagging machines were adequately trained in the skills required for the safe application, usage, and removal of energy controls.  Employees that setup, unjam, and repair the equipment were not trained to control the machine's hazardous energy.

Jones Potato Chip Co. - Mansfield, Ohio: On or about December 6, 2017, the employer did not ensure that the #4 Hayssen bagging machine (s/n 086013) was guarded to protect employees from rotating parts and pinch points.  Employees operating adjustment points were exposed to being caught in the machine's rotating parts and pinch points created by the closing sealer jaws.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours:    a. Jones Potato Chip Co. - Mansfield, Ohio: On or about December 26, 2017, the employer failed to report an amputation injury to an employee.  On December 6, 2017, an employee sustained a partial finger amputation injury; the employer did not report the amputation within twenty-four hours.
