Failure to ensure employees are adequately trained to operate powered industrial vehicles exposes employees to potential struck by hazards and amputation hazards.

Failure to ensure powered industrial vehicle drivers operate trucks with extremities inside the running lines of the truck exposes employees to struck by hazards and amputation hazards.

On or about April 12, 2023, the employer failed to provide the OSHA 300 Log of Wok-Related Injuries and the OSHA 300A Summary of Work-Related Injuries and Illnesses for calendar year 2022.
