29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  Building 3703A, warehouse receiving area; on or about 02 January 2018 and times prior thereto; two instances were noted where the walking-working surfaces were not maintained free of hazards:       a. Throughout the warehouse; Glycol is leaking from overhead hydronic heating lines onto the walking-working surface below creating slippery surface conditions. Multiple locations throughout the warehouse have a variety of collection containers dispersed in an attempted to contain the leakage. Containers are routinely moved to allow forklift operator's clear passage to position warehouse pallets. Prediction of drip patterns makes positioning of collection containers ineffective.        b. Loading ramp areas; Ice and snow accumulation on the ramp and loading dock trailer stalls create uneven and slippery surface conditions. Loading dock trailer stalls have significant ice and snow accumulation and creates a condition where trailers and dock bridge plates are not level.  This condition exposed employees to a fall hazard.

29 CFR 1910.22(d)(1): The employer did not ensure that walking-working surfaces were inspected, regularly and as necessary, and maintained in a safe condition.  Building 3703A, corridor to meat preparation area; on or about 02 January 2018 and times prior thereto; a floor hole measuring approximately four and one-half inches in diameter, with a depth of approximately two inches was not effectively  protected. The access cover was missing and the floor hole was covered with duct tape.   This condition exposed employees to a tripping hazard.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems.  Building 3703A, warehouse area; on or about 02 January 2018 and times prior thereto; two instances were noted where unprotected edges were not guarded:       a. Receiving dock; the receiving dock measuring 48-inches from the lower surface was not provided with guardrails. Area is primarily used to unload trailers using pallet jackets.       b. Mechanical chase room; an 18-inch opening with access to a fixed ladder, located 72-inches above the lower surface was not protected.   This condition exposed employees to a fall hazard.
