29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards of nine feet as they were working on the sixth floor of the Townhouse building passing up materials in close proximity to the building perimeter of the southeastern stair tower elevation without being tied off or having guardrails in place. Violations were observed on or about December 8th, 2017 and December 11th, 2017. b) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards of nine feet as he was working on top of a wall on the south side of the sixth floor of the building setting up boards to support the plywood sheeting for the seventh floor without being tied off or having guardrails in place. Violations were observed on or about December 8th, 2017. c) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards of nine feet as he was being handed up materials from the ground and handing them up to another employee on the third floor in close proximity to the building perimeter on the southwest side of the second floor without being tied off or having guardrails in place. Violations were observed on or about December 8th, 2017. d) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards of nine feet as they were being handling forms and materials in close proximity to the building perimeter on the southeast side of the sixth floor without being tied off or having guardrails in place. Violations were observed on or about December 8th, 2017. e) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards of nine feet as he was overseeing the scaffold installation in close proximity to the building perimeter on the southwest side of the sixth floor without being tied off or having guardrails in place. Violations were observed on or about December 8th, 2017. f) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards of nine feet as he was receiving a crane load of forms in close proximity to the building perimeter on the southeast side of the fifth floor without being tied off or having guardrails in place. The open holes were over uncapped vertical rebar on the next lower level approximately nine feet down. Violation was observed on or about December 11th, 2017. b) Worksite was located at 235 Grand Street, Jersey City, NJ 07302 Employees were exposed to fall hazards as there were open holes approximately two feet by two feet in the flooring where employees were doing layout on the seventh floor above a concrete floor on the next lower level approximately nine feet down.
