29 CFR 1926.502(i)(2): All other covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. A) Pro Fit HVAC, LLC on or about December 1, 2017, at the address of 370 Eastown Road, Lima, Ohio: The employer did not ensure the styrofoam sheets that were used to cover 4 foot x4 foot mechanical equipment access holes used for placement of HVAC systems, were able support twice the weight of the employees and equipment. One employee stepped on a cover which failed, resulting in the employee falling 10 feet to the level below.
