(SEE FIELD INFORMATION MEMORANDUM 75-25)   a) Warehouse - On or about January 5, 2018 and at times prior thereto, employees operate forklift trucks inside of highway trucks to unload them, while the highway trucks wheels are not chocked to prevent rolling.   On January 3, 2018 a highway truck rolled away form the loading dock while being unloaded causing the forklift to fall from the truck to the ground.
